Understanding Your Concern
Why You Look Tired When You Are Not
The tear trough is the groove running from the inner corner of the eye down along the top of the cheekbone. As the fat pads supporting the mid-face lose volume and drift downwards with age, that groove deepens into a visible hollow. Light no longer falls evenly across the area — it casts a shadow instead, and the result reads as tiredness.
Men tend to notice it in photographs before they notice it in the mirror, usually somewhere in their late thirties or forties. The common complaint is consistent: looking drained in every work photo, being asked whether everything is alright, appearing far more tired than they feel.
Not every dark under-eye is a hollow. Some are pigmentation, where the skin itself is darker. Some are visible blood vessels showing through thin skin. Some are true fat herniation — the protruding pads commonly called eye bags — where adding filler makes the area look heavier rather than fresher. Only the first responds well to this treatment, and distinguishing between them is the entire point of the consultation.
Our Treatment
How the Treatment Works
Treatment uses a small quantity of soft hyaluronic acid dermal filler placed deep beneath the skin, against the bone, to restore support where volume has been lost. As the hollow fills, the shadow lifts and the area reflects light more evenly.
Technique matters more here than anywhere else on the face. The skin beneath the eye is the thinnest on the body, the area is rich in blood vessels, and product placed too superficially or in too great a quantity produces a bluish tinge or persistent puffiness that can last months. A cannula is generally used rather than a needle — a blunt-tipped instrument that moves between tissue planes rather than through them, reducing the risk of bruising and vessel damage.
Volume. Very little product is used — commonly under one millilitre across both sides. Underfilling and reviewing after two weeks is the correct approach in this area. Any practitioner offering to fill your tear troughs generously in a single sitting is taking a risk with your face.
The appointment. Treatment takes around 30 minutes. Topical anaesthetic is applied first and the filler contains local anaesthetic. Most men describe pressure rather than pain.
Downtime & Results
Downtime and Results
Bruising is more likely here than in any other area of the face, and it can be noticeable for a week or more. Swelling in the first few days is normal and often uneven between sides. If you have something significant in the diary, allow at least two weeks.
Sleep with your head slightly elevated for the first few nights and avoid the gym, saunas, steam rooms and alcohol for 24 to 48 hours.
Judging the result. The improvement is often visible immediately, but what you see in the first week includes swelling. The final result settles at around two to four weeks, and a review appointment is where any adjustment gets decided.
Results typically last nine to eighteen months. Filler in this area breaks down slowly because there is very little muscle movement to accelerate it.
Some men experience persistent puffiness in the weeks after treatment, caused by the filler attracting water in an area that drains poorly. Where that happens and does not settle, the filler can be dissolved — a prescription-only medicine administered in clinic by Nurse Kerry, our Independent Nurse Prescriber.
“This is the area where restraint matters most. Less product, placed deeply, reviewed after a fortnight — and an honest no where the cause is pigmentation or fat herniation rather than volume loss.”
Suitability
Who Is This Treatment Suitable For?
This treatment suits a narrower group than most. It is not suitable where the darkness is caused by pigmentation, where there is significant fat herniation, where there is marked skin laxity or fluid retention beneath the eyes, or where you have a history of puffiness in that area. Assessment is in person. It generally suits:
- Genuine hollowing beneath the eye
- Shadowing that worsens in overhead light
- Good skin quality with little laxity
- Looking tired in photographs
- Early to moderate volume loss
- Not suitable for pigmentation-related darkness
- Not suitable for true eye bags
- Filler dissolving available in clinic

Your Questions Answered
Frequently Asked Questions
Only if they are caused by hollowing. Darkness from pigmentation in the skin, or from blood vessels showing through thin skin, will not improve with filler — and might look slightly worse. The cause is assessed in person before any treatment is agreed, and you may be recommended something else entirely.
Typically nine to eighteen months. Filler in this area breaks down slowly because there is very little muscle movement to accelerate the process.
Quite possibly. This area bruises more readily than anywhere else on the face and it can be noticeable for a week or more. Allow at least two weeks before anything significant in the diary.
Topical anaesthetic is applied first and the filler contains local anaesthetic. Most men describe pressure rather than pain. Treatment takes around 30 minutes.
Some men do — the filler attracts water and this area drains poorly. It often settles within a few weeks. Where it does not, the filler can be dissolved. That is a prescription-only medicine and Nurse Kerry, our Independent Nurse Prescriber, carries it out in clinic.
Generally no. True eye bags are herniated fat pads pushing forward, and adding filler beneath them makes the area heavier rather than fresher. That is a surgical matter and you would be told so at consultation rather than treated anyway.
Very little — commonly under one millilitre across both sides. Underfilling and reviewing after a fortnight is the correct approach here. Overfilling the tear trough is the most common way this treatment goes wrong.
Not for 24 to 48 hours. Avoid the gym, saunas, steam rooms and alcohol, and sleep with your head slightly elevated for the first few nights to reduce swelling.
